Any request for warranty repair service must include the information on this form:
Name of client filing the warranty claim: MMMMMMMMMMMMMMMMMM

Serial Number of the product under warranty: MMMMMMMMMMM
Date the warranty product was purchased by the client: Day MM Month MM Year MMM
Date the warranty product was recieved by the client: Day MM Month MM Year MMM
This information will be sent to Aaron@pangolin.com and Adam@pangolin.com for approval
Aaron is responsible for all final judgements regarding any warranty claim submitted to Pangolin and allowed under Pangolin’s Terms of Service, Product Warranty section.
Adam will be validating warranty claims per Pangolin’s Terms of Service, Product Warranty section.
All clients requesting repairs from Pangolin Laser Repair must have a completed Service Registeration.
